No you do not need to own the original. Pandemic Legacy recommends some prior experience with Pandemic, but the rule book in Pandemic Legacy explains how to gain this experience with a few trial games using everything in the new box.

As others have said, the Pandemic system gets harder with more players. But this one is especially brutal in March and April (or maybe it was April and May) then gets way easier again. The system itself is also a lot more random than other Pandemics because of having multiple cards for each city in the infection deck.
